General Description

As Quality Metric Coordinator, you will be responsible for ensuring accurate and timely reporting of healthcare quality metrics. This role involves analyzing and interpreting healthcare data, collaborating with cross-functional teams, implementing strategies to improve and maintain high-quality standards in healthcare reporting, and development of process improvement plans to reduce open care gaps. An ideal candidate will support our mission to provide quality health and wellness services where the patient, family, and community come first.

Essential Job Duties

  • Conduct prospective chart preparation for clinical diagnostic and quality opportunities in advance of patient visit to ensure a plan to address open care gaps
  • Interact with healthcare professionals to provide report-specific feedback and assist in resolving discrepancies
  • Engage with leadership and care team members to ensure patients are prioritized for scheduling based on outstanding care gaps
  • Contact patients to assist with medication adherence and preventative screening scheduling
  • Collaborate with Accountable Care Organization (ACO) groups and various insurance providers to identify and close gaps in patient care
  • Collaborate with cross-functional teams to gather, validate, and integrate data from various sources
  • Generate comprehensive reports to measure and track performance metrics related to healthcare quality measures
  • Data entry of health plan quality metrics
  • Support and facilitate implementation of performance improvement activities resulting from identification of care gaps
  • Stay informed of changes in quality reporting requirements and implement necessary adjustments
  • Maintain strict confidentiality and adhere to all HIPAA guidelines and regulations
  • Perform other related duties as assigned

Minimum Qualifications

  • One (1)+ years’ experience within a healthcare setting
  • Knowledge of clinical nursing practices and medical terminology
  • Understanding of reportable quality metrics
  • One (1)+ years’ experience abstracting and utilizing information from electronic medical records, documents, and related information
  • Proficiency in using Microsoft Office programs and web applications
  • Ability to analyze data, share information and make recommendations for improvements
  • Numerical, analytical, and investigative skills
  • Ability to work in both independent and collaborative settings
  • Demonstrate time management skills and ability to adhere to project productivity metrics and time lines
  • Strong interpersonal and communication skills
  • Strong customer service focus

Preferred Qualifications

  • Licensed Practical Nurse (LPN)
  • Registered Nurse (RN)
  • Continuous quality improvement experience
  • Care management experience
  • Bilingual (English/Spanish)
  • Healthcare Effectiveness Data and Information Set (HEDIS) quality reporting experience
  • Other quality metrics abstraction and reporting experience

Physical Requirements / Working Conditions

  • Must be able to sit for long periods of time
  • Must be able to stand, walk, stoop, bend, squat, and stretch for portions of the day
  • Must be able to lift and carry up to twenty (20) pounds
  • Must be able to operate standard office equipment
