General Description:

The 340B Referral Closure Specialist ensures the proper management and follow-up of referrals related to the 340B Drug Pricing Program. This role is responsible for “closing the loop” on 340B referrals by verifying documentation, ensuring timely communication with stakeholders, and ensuring compliance with program requirements. This position works closely with internal departments, internal and external providers and pharmacies to support the accurate tracking and reporting of 340B-related referrals. This position is integral to ensuring compliance with 340B program guidelines and supports the Health Information Management (HIM) department by maintaining accurate records and documentation.

Essential Duties:  

          Referral Tracking and Closure Follow-Up:

  • Monitor and track all referrals to ensure timely resolution and that proper documentation is received from external providers.

  • Close the loop by verifying that all referral outcomes are documented and accessible in the electronic medical record (EMR) system.

  • Follow up with internal departments, pharmacies and other healthcare providers to confirm referral outcomes and assist with closing the referral loop.

  • Escalate unresolved referral issues to HIM Manager.

          Compliance and Documentation:
  • Ensure all referral-related documentation complies with 340B program guidelines, organizational policies, and federal regulations.

  • Regularly audit referral documentation for accuracy and completeness.

  • Collaborate with the HIM team to ensure consistent and compliant recordkeeping practices.

          Communication and Coordination:
  • Serve as the primary point of contact for 340B referral-related inquiries.
  • Liaise with external providers, pharmacies, and internal clinical staff to resolve documentation issues and ensure timely follow-up.
  • Provide updates to the HIM Manager on referral trends and compliance concerns.

          Program Support and Reporting:

  • Support the HIM Manager and pharmacist in preparing reports related to referral closure and program performance.
  • Identify gaps or inefficiencies in the referral closure process and propose solutions for improvement.
  • Assist with audits or reviews of 340B program compliance as requested.

 Other Duties

  • Maintain a referral tracking system or database specific to 340B activities.
  • Maintain accurate and organized records of 340B program activities.
  • Handle sensitive patient information in accordance with HIPAA and other applicable regulations.

Qualifications: 

  • High school diploma or equivalent required; some college coursework in healthcare administration, health information management, or a related field preferred.

Preferred Qualifications

  • Minimum of 1-2 years in healthcare, referral management, or medical records; familiarity with the 340B program is highly preferred.

Physical Requirements/ Working Conditions:

  • Basic understanding of 340B program guidelines and compliance requirements (or willingness to learn).
  • Excellent organizational skills and attention to detail.
  • Proficiency in electronic medical record (EMR) systems and Microsoft Office applications.
  • Strong communication and interpersonal skills.
  • Ability to work independently and prioritize tasks in a fast-paced environment.
  • Knowledge of HIPAA regulations.
  • Proactive approach to identifying and resolving issues.
  • Willingness to learn and adapt to new tools and processes.
